They are many reasons why this happens but we can avoid this scenario with better leadership by

➡️ Re-focusing risk management; and

➡️ Targeting continuous improvement and transition efforts towards the Operating Model that we want.

Achieving that Operating Model shall involve the leadership that transitions folk from their comfort zone, through the fear zone, and on to the learning and growth zones.
